Travel insurance for a trip to Pakistan: cash-first hospitals, advisories and what to buy

Buy a single-trip emergency medical policy for every traveller before a trip to Pakistan. Provincial plans pay $75 to $400 a day for hospital care abroad and nothing for a flight home. The Government of Canada says good care is limited to a few hospitals in Islamabad, Karachi and Lahore, emergency and ambulance services are virtually non-existent in most of the country, and most facilities require cash up front. Check the regional advisories against the policy wording.

Buy a single-trip emergency medical policy for every traveller before a trip to Pakistan. Provincial plans pay $75 to $400 a day for hospital care abroad and nothing for a flight home. The Government of Canada’s Pakistan advisory of 15 October 2026 says good care is limited to a few hospitals in Islamabad, Karachi and Lahore, emergency and ambulance services are “virtually non-existent in most of Pakistan,” and most facilities require cash prepayment. Check the regional advisories against the policy wording before booking, because they cover large parts of the country.

Pakistan is the third-largest source of super visa parents at 5%, and the typical family trip runs four to eight weeks over a wedding or Eid, often with a parent who is now a permanent resident and sometimes one still on a super visa. The advisory map, not the hospital bill, is what makes Pakistan different from India.

What does your provincial plan pay in Pakistan?

Fixed daily rates after you pay, in Canadian dollars. Ontario’s Out-of-Country Travellers Program pays up to $400 a day for operating-room, intensive, coronary or neonatal care, $200 a day for lower levels, $50 a day for emergency outpatient care and physician fees at Ontario rates, per Ontario’s page. Alberta pays up to $100 a day inpatient and $50 outpatient, in Canadian funds only, and does not reimburse private facilities, per Alberta’s page.

Cost in Pakistan Ontario pays Alberta pays Private policy pays
Cash deposit demanded at admission $0 at the time $0 at the time Payment guarantee via assistance line, at major hospitals
Private hospital ward, per day Up to $200 Up to $100 Actual cost to the policy limit
ICU, per day Up to $400 Up to $100 Actual cost to the policy limit
Private facility Reimbursed at the daily cap Not reimbursed Covered
Air ambulance to Canada $0 $0 Covered subject to the insurer’s medical approval
Return of remains $0 $0 Covered to the policy limit

Most private hospitals that Canadian families use in Lahore, Karachi and Islamabad are private facilities, which Alberta will not reimburse at all. See what provincial plans pay abroad for every province.

Which regions does the advisory cover, and how do policies treat them?

Large parts of the north and west are under avoid-all-travel advisories, and Karachi is under avoid-non-essential-travel except named districts. Canadian policies commonly exclude claims arising in a region under an avoid-all-travel advisory issued before departure, so the advisory is in effect part of the policy.

Region Advisory, 15 Oct 2026 Typical policy treatment
Punjab (Lahore, Faisalabad, Gujranwala, Sialkot) High degree of caution Covered
Islamabad High degree of caution Covered
Karachi: Clifton, listed DHA phases, airport corridor High degree of caution Covered
Karachi: rest of city Avoid non-essential travel Usually covered; confirm
Balochistan Avoid all travel Commonly excluded
Khyber Pakhtunkhwa, except Haripur and Abbottabad (high caution) and Chitral (avoid non-essential) Avoid all travel Commonly excluded
Pakistan-administered Kashmir Avoid all travel Commonly excluded
Within 50 km of the Afghan border; within 10 km of the Indian, Chinese and Iranian borders and the Line of Control, except Wagah, Khunjerab and Kartarpur crossings Avoid all travel Commonly excluded
Karakoram Highway, Mansehra to Chilas Avoid all travel Commonly excluded

Source: Government of Canada, Pakistan. A family whose village is in Sialkot district near the border, or who plans a drive to Hunza on the Karakoram Highway, needs the carrier’s wording on regional advisories read before departure, or accepts that costs there are self-funded. Wagah, Khunjerab Pass and the Kartarpur Corridor are carved out as crossings.

Why does evacuation matter more in Pakistan?

Because the advisory says ambulance services barely exist outside the big cities, and the private policy’s assistance company is the only organization that will move a patient. The Government of Canada’s travel insurance page puts medical evacuation with a medical escort first on its list of what a policy must cover, and ITIJ quotes an operator putting a long-haul flight to the patient’s home country at up to US$150,000. A stroke or heart attack in a village in Gujrat district is first a question of reaching Lahore, then a question of whether to fly home. The policy pays for both decisions.

Single-trip or annual?

Single-trip for a four- to eight-week visit; annual only if the family also makes short trips. BMO’s guide gives annual plans per-trip limits of 4, 10 or 23 days and warns that an overrun without a top-up can suspend coverage for the whole trip. A wedding trip that stretches from five weeks to seven is a single-trip extension, requested before the original end date; see annual multi-trip plans.

Who in the family needs which policy?

Everyone, matched to their status, with the super visa parent the one most often uninsured.

Traveller Policy Note
Host and spouse under 55 Single-trip, no questionnaire Name Pakistan and transit stops (Dubai, Doha, Istanbul, Abu Dhabi)
Canadian-born children Added to the family policy Confirm infant minimum age
Parent who is a PR, 60 to 75 Single-trip with questionnaire 120 to 180 days stability; watch provincial presence rules
Parent on a super visa Separate single-trip for Pakistan Allianz keeps the policy in force in the country of origin but pays nothing for costs there, per its certificate; see mid-stay trips
Pilgrim continuing to Hajj or Umrah Single-trip naming Saudi Arabia and Pakistan Saudi visa insurance is not enough; see Hajj and Umrah
Grandparent over 70 Single-trip with questionnaire See over-70 travellers

Carriers include Manulife CoverMe (Manufacturers Life), TuGo (iA Financial Group), Allianz Global Assistance (CUMIS General Insurance), GMS (Group Medical Services) and Destination Travel Group (Zurich Insurance Company Ltd, Canadian Branch). Stability periods run 90 to 365 days depending on age and carrier; verify at quote time. If someone is admitted, call the assistance line before treatment or within 24 hours and keep every receipt, in English or translated; the claims guide has the sequence.

Questions people ask

Does OHIP cover me in Pakistan?

At fixed daily rates only: up to $400 a day for intensive or operating-room care, $200 for a ward and $50 for outpatient emergencies, plus physician fees at Ontario rates, claimed after you have paid. OHIP does not cover transferring you to an Ontario hospital. Other provinces pay $75 to $100 a day. A private policy covers the rest.

Do Pakistani hospitals take insurance?

Most require cash prepayment, per the Government of Canada's advisory. A travel medical policy's assistance line can issue a payment guarantee to a major hospital in Islamabad, Karachi or Lahore; smaller facilities will still want cash. Keep enough on a card to cover a deposit and claim it back with receipts.

Will travel insurance pay for a claim in Khyber Pakhtunkhwa or Balochistan?

Usually not, and check the wording. As of 15 October 2026 the Government of Canada advises avoiding all travel to Balochistan, most of Khyber Pakhtunkhwa, Pakistan-administered Kashmir, areas within 50 km of the Afghan border and 10 km of the Indian, Chinese and Iranian borders, and the Karakoram Highway from Mansehra to Chilas. Policies commonly exclude claims in avoid-all-travel regions.

Is Karachi covered?

Parts of it. The advisory says avoid non-essential travel to Karachi except specified areas such as Clifton, several DHA phases and the airport corridor, where a high degree of caution applies. Avoid-non-essential-travel regions are usually covered; confirm the carrier's position on Karachi when buying, and name the city on the application.

What about evacuation from Pakistan?

Air ambulance to Canada is the largest single exposure, and no provincial plan pays for it. The Government of Canada says medical evacuation can be very expensive and that insurance should cover it. The policy's assistance company decides whether to stabilize in Lahore or Islamabad or fly the patient home; without a policy that decision is the family's, at its own cost.

My father is on a super visa and travelling with us to Lahore. Does his policy cover him there?

Usually not. Allianz keeps the policy in force in the country of origin but pays nothing for costs incurred there; TuGo excludes expenses in the home country; Manulife excludes the home country from side trips. Buy him a separate single-trip policy for Pakistan, and renew the super visa policy so a full year shows on his return to Canada.
