Travel insurance for a trip to India: what OHIP pays, what to buy, and what to check

Buy a single-trip emergency medical policy for every traveller before a trip to India. OHIP pays at most $400 a day for hospital care abroad and nothing for a flight home; Indian hospitals usually want payment or insurance confirmation before treatment. Hospital bills in India are modest by Canadian standards, but an air ambulance to Canada is not. Over-55s and anyone on medication need the questionnaire done before the pre-trip clinic visit.

Buy a single-trip emergency medical policy for every traveller before a trip to India. OHIP pays at most $400 a day for hospital care abroad and nothing for a transfer home, per Ontario’s page, and the Government of Canada’s India advisory says most hospitals require up-front payment or confirmation of insurance before treatment. Hospital bills in India are modest by Canadian standards; an air ambulance to Canada is not. Over-55s and anyone on medication need the questionnaire completed before the pre-trip clinic visit.

India accounts for 56% of super visa parents, so most families reading this will travel with a mix of Canadian citizens, permanent residents and a parent on a visitor status. Each needs a different policy; the table at the end sorts them.

What does OHIP actually pay in India?

Fixed daily amounts, in Canadian dollars, after you pay the hospital and claim. OHIP’s Out-of-Country Travellers Program pays up to $400 a day for care in an operating room, intensive care, coronary or neonatal unit, $200 a day for lower levels of care, $50 a day for emergency outpatient care, and physician services at the lesser of the billed amount or the Ontario Schedule of Benefits rate.

Cost in India OHIP pays Private policy pays
Private hospital ward, per day Up to $200 Actual cost to the policy limit
ICU, per day Up to $400 Actual cost to the policy limit
Surgeon and specialists Ontario schedule rate Actual cost
Emergency outpatient visit Up to $50 Actual cost
Air ambulance to Canada $0 Covered subject to the insurer’s medical approval
Transfer to an Ontario hospital $0 (“OHIP also does not cover transferring you to an Ontario hospital”) Covered as part of evacuation
Return of remains $0 Covered to the policy limit

Why is evacuation the real exposure?

Because Indian hospital prices are low and air ambulance prices are not. A corporate hospital in Delhi, Mumbai, Chandigarh or Hyderabad charges a fraction of a Canadian hospital for a room, surgery or ICU, which is why some families consider self-funding. The number that breaks that plan is the flight: ITIJ quotes an operator putting a flight to the patient’s home country at up to US$150,000, against about US$25,000 to the nearest centre of excellence.

The Government of Canada’s travel insurance page lists medical evacuation to Canada or the nearest appropriate facility, with a medical escort, as the first thing a policy must cover. It also notes the Government of Canada will not pay your medical bills. For a stroke in a village in Punjab, the policy’s assistance company decides whether to stabilize locally, move to a city hospital, or fly home; without a policy the family makes that decision with its own money.

Single-trip or annual multi-trip?

Single-trip for the typical four- to eight-week visit; annual only for a family that also makes several short trips. BMO’s guide gives annual plans per-trip limits of 4, 10 or 23 days depending on the plan, with top-ups available if bought before the base coverage ends, and warns that exceeding the limit without a top-up can suspend coverage for the whole trip.

Trip pattern Buy Watch
One trip to India, 4 to 8 weeks Single-trip, exact dates Extend before the end date if the return is delayed
India for 3 weeks, plus Buffalo and Florida Annual with 23-day limit Every trip must fit the limit
India for 6 weeks, plus short US trips Annual plus a top-up for the India trip Buy the top-up before departure
India for 5 months (PR parent) Single-trip, long duration Ontario’s 153-day presence rule; see PR parents

What do the carriers require for pre-existing conditions?

A stability period before departure, longer with age. Allianz’s certificate requires 90 days of stability under 60 and 180 days from 60 to 89, and defines stable as no new treatment, change in treatment, new symptoms, deteriorating results, hospitalization or specialist referral, per its certificate. Our carrier fact sheet records the others.

Carrier (underwriter) Stability period Max age
Allianz Global Assistance (CUMIS General Insurance) 90 days under 60; 180 days 60 to 89 89
TuGo (iA Financial Group) 90 days to 59; 120 days 60 to 69; 180 days 70 to 85; 365 days 86+ None
Manulife CoverMe (Manufacturers Life) 180 days 85
GMS (Group Medical Services) 180 days; no medical questions under 55 79
Destination Travel Group (Zurich Insurance Company Ltd, Canadian Branch) 90 days 0 to 59; 120 days 60 to 69; 180 days 70 to 79 About 90; pre-existing excluded 80+

Which regions are excluded, and what about dengue?

Regions under an avoid-all-travel advisory at purchase are commonly excluded from claims; dengue and malaria are covered as emergencies if they arise on the trip. The India advisory of 14 October 2026 says to exercise a high degree of caution countrywide, avoid all travel to Jammu and Kashmir (not Ladakh) and to areas within 10 km of the Pakistan border in Gujarat, Punjab and Rajasthan, and avoid non-essential travel to Manipur. Many Punjabi families have villages inside that 10 km band; check the policy wording for how it treats regional advisories before relying on coverage there.

Dengue is a risk throughout India, with no vaccine or preventive medication available in Canada, and malaria is a risk in some areas and seasons, per the same advisory. Both are sudden illnesses on the trip and are covered as emergencies; the pre-trip travel clinic visit, about six weeks out, is also when the questionnaire should already be done.

Who needs which policy on a family trip to India?

Every traveller, with the policy matched to their status.

Traveller Policy Note
Host and spouse, Canadian residents under 55 Single-trip, no questionnaire Name India and any transit stop
Children born in Canada Added to the family policy Confirm infant minimum age
Parent who is a permanent resident, 60 to 75 Single-trip with questionnaire 120 to 180 days stability; watch the 153-day OHIP rule
Parent on a super visa travelling with you Separate policy for India The super visa policy may not pay in the home country; see mid-stay trips
Grandparent over 70 Single-trip with questionnaire See over-70 travellers

If someone is admitted, call the assistance line before treatment or within 24 hours; Allianz, for example, pays only 80% of eligible expenses when it is not notified without reasonable cause. The claims guide has the sequence.

Questions people ask

Does OHIP cover me in India?

Only at fixed daily rates: up to $400 a day for intensive or operating-room care, $200 a day for a ward, $50 a day for emergency outpatient care, plus physician fees at Ontario rates, claimed after you have paid. OHIP does not cover a transfer back to an Ontario hospital. A private policy covers the rest.

Is travel insurance to India expensive if hospital care there is cheap?

The premium reflects the whole risk, including evacuation. Care at a private hospital in Delhi or Mumbai costs far less than in Toronto, but an air ambulance to Canada can run to six figures and is excluded from every provincial plan. Premiums depend on age, trip length, deductible and pre-existing conditions; confirm at quote time.

Single-trip or annual plan for a six-week trip to Punjab?

Single-trip. Annual multi-trip plans cap each trip at a set number of days, commonly 4, 10 or 23, and a trip that overruns without a top-up can lose coverage for the whole trip. A six-week visit exceeds every standard annual limit. Buy annual only if the family also makes several shorter trips a year.

Will my parents' diabetes or blood pressure be covered on a trip to India?

Yes if the condition has been stable for the carrier's period before departure: 90 days under 60 at Allianz and TuGo, 120 days at 60 to 69 at TuGo and Destination, 180 days at 70 and over at most carriers and at Manulife and GMS for all ages. A medication change inside the window makes the condition unstable, so buy before the pre-trip doctor's visit.

Are there parts of India where insurance will not pay?

Policies commonly exclude claims arising in regions under a Government of Canada avoid-all-travel advisory issued before departure. For India that is Jammu and Kashmir (not Ladakh) and areas within 10 km of the Pakistan border in Gujarat, Punjab and Rajasthan, as of 14 October 2026. Check the policy wording and the advisory on the day you buy.

What do I do if someone is admitted to hospital in India?

Call the insurer's assistance line before treatment, or within 24 hours of an emergency admission, and before any surgery. The assistance company confirms coverage, arranges a payment guarantee with the hospital where possible, and decides on evacuation. Keep every receipt, report and discharge summary, in English or translated.
