You apply for OHIP in person at a ServiceOntario centre with three original documents, one each for immigration status, Ontario residency and identity, plus a completed registration form. There is no waiting period; the OHIP page says coverage starts immediately for eligible people. The steps below take most newcomers one to three weeks, with the residency document usually the slowest part.
Before you start
Check eligibility first. You need Ontario as your primary residence, physical presence for 153 of the first 183 days after you begin living in Ontario, and a qualifying status. The qualifying statuses are citizen, permanent resident, applicant for PR whom IRCC has confirmed as eligible, protected person, registered Indigenous person, specific Temporary Resident Permit case types, and work permit holders “working full-time in Ontario, for an Ontario employer, for at least 6 months.” Visitors and super visa holders never qualify. Our Ontario eligibility page has the full table.
Have ready:
- Immigration status document (original): PR card, confirmation of permanent residence, work permit, protected person document or passport with citizenship proof.
- Ontario residency document (original): lease, utility bill, bank statement with address, or employer letter showing your Ontario address.
- Identity document (original): passport, driver’s licence or other photo ID.
- Employer letter, if your work permit does not state full-time hours, the Ontario employer and a six-month term.
- The Registration for Ontario Health Insurance Coverage form, completed for each family member.
Time needed: a 20-to-60-minute ServiceOntario visit, after one to two weeks assembling documents. The card arrives by mail afterwards.
Steps
- Confirm your qualifying status against the OHIP page. If you are a work permit holder, read the permit: it must show, or an employer letter must show, a full-time job for an Ontario employer of at least six months. Steps to Justice notes an employer letter is accepted where the permit is silent.
- Secure an Ontario residency document. Sign the lease or set up a utility account in your own name as early as possible; this is the document newcomers most often lack on day one.
- Download and complete the registration form for each person applying. Spouses and children of a qualifying worker apply on their own forms with their own status documents.
- Buy a short private plan if you do not already have one, running from arrival to the expected card date. GMS and TuGo waive the illness waiting period only when the policy is bought before arrival; after arrival, buy today rather than tomorrow.
- Find your nearest ServiceOntario centre and check whether it takes appointments. The province’s page requires an in-person visit and does not mention booking; some centres run walk-in only.
- Attend with every original document for every family member. Photocopies are refused, and a missing original means a second visit.
- Ask the clerk to confirm your eligibility date before you leave. That date is when coverage begins, and it is the date you will give any clinic that billed you in the meantime.
- Watch for the health card by mail. Once it arrives, give the number to any provider who treated you after the eligibility date and ask them to bill OHIP or refund you.
- Diarize the 153-day rule. Do not leave Ontario for more than 30 days in your first six months, or the first residency condition fails and coverage can be cancelled.
Work permit holders: the three conditions
Full-time, Ontario employer, six months. All three must hold at the time of application. A worker on an open permit who has not yet found a job does not qualify until they have one; a worker whose job is in Gatineau does not qualify in Ontario; a worker with a four-month contract does not qualify. In each case the worker holds a private permit-holder plan until the condition is met, then applies. Our work permit page compares the plans, and dependants of work permit holders covers the family.
| Permit situation | OHIP eligible now? | What to do |
|---|---|---|
| Closed permit, Ontario employer, 12 months, full-time | Yes | Apply on arrival |
| Open permit, job not yet started | Not yet | Private plan; apply with employer letter once working full-time |
| Open permit, two part-time jobs | No | Private plan; OHIP requires full-time with one Ontario employer |
| Closed permit, 5-month contract | No | Private plan for the stay; reapply if extended to 6+ months |
| Spouse of a qualifying worker, on a visitor record | Yes, as a dependant | Apply with the worker, with own status and identity documents |
If something goes wrong
Refused at the counter: ask which of the three residency conditions or the status test failed, and get it in writing if possible. A refused residency document is fixed by returning with a lease or utility bill. A refused permit is fixed by an employer letter, or by waiting until the job meets the test. A refusal for absence from Ontario is fixed only by time.
Treated before the card arrived: keep every invoice. Ask the hospital to rebill OHIP once you have the number and the eligibility date. If the care was before the eligibility date, OHIP will not pay it; that is what the private plan in step 4 is for. Queensway Carleton in Ottawa bills $414 for an emergency visit and $1,335 a ward day at the uninsured-resident rate, and more at the non-resident rate.
A family member who does not qualify: a parent on a super visa, a student spouse without a qualifying job, a child over 21 who is not disabled. They are not OHIP-eligible and need a visitor plan for the stay, and in the super visa case a $100,000 one-year policy that IRCC requires; see super visa insurance.
SuperVisaDirect is operated by Super Visa Direct Insurance Services, an independent Ontario life and accident-and-sickness insurance agency licensed by FSRA (licence [FSRA-LICENCE]), serving Ontario residents. We sell the short bridge plan in step 4 and visitor plans for family members who do not qualify. See the pre-arrival checklist for what to do before the flight.
Questions people ask
Can I apply for OHIP online?
No. Ontario's OHIP page says you must apply in person at a ServiceOntario centre with original documents; photocopies are not accepted. Check whether your local centre takes appointments, and bring every original the same day so the visit is not wasted.
Which three documents do I need?
One proving Canadian citizenship or an OHIP-eligible immigration status, such as a PR card, confirmation of permanent residence or a work permit; one proving Ontario residency, such as a lease, utility bill or employer letter with your address; and one proving identity, such as a passport or driver's licence. Each must be an original and each must be a different document.
What must a work permit show?
That you are working full-time in Ontario, for an Ontario employer, for at least six months. If the permit does not state the employer or hours, bring an employer letter that does. An open permit with a part-time or out-of-province job does not qualify, and the spouse and dependants qualify only through a qualifying worker.
When does coverage start?
From the date you qualify. There is no longer a waiting period for OHIP. In practice you are covered from the eligibility date ServiceOntario confirms, and claims from that date onward can be submitted once the card is issued. Care before the eligibility date is not covered.
What if ServiceOntario refuses my application?
Ask which condition failed and in writing. The common reasons are a permit that does not show a full-time, six-month Ontario job, a missing residency document, or an absence of more than 30 days in the first six months. Fix the document, or reapply once the condition is met, and hold private cover in the meantime.
Does my parent on a super visa need to apply?
No, and the application would be refused. Visitors from outside Canada do not qualify for OHIP, and a super visa holder is a visitor for the whole stay. The parent needs a private visitor plan from a Canadian insurer with at least $100,000 of coverage for a year from entry, renewed each year.
